Personal - Hospital, Surgical & Inpatient, Day Surgery
Prime Care , Premier Care & MumsCare/MumsCare Plus Claims
Please follow the STEPS below for Hospitalization Claim (applicable for Prime Care, Premier Care, MumsCare/MumsCare Plus policies).

Step 1

Complete Claim Form

  • Submit the claim within 30 days from the event date.
  • Complete Claim Form
  • Submit Claim Form with supporting documents (refer to Step 2)



Step 2
Submit Supporting Documents

Please note that the list of documents below is not exhaustive and that other documents may be requested if necessary.

Prime Care Policy
Please submit:
  • Daily Hospital Cash Benefit including Daily Room & Board and Intensive Care Unit (ICU), and/or
  • Surgical Benefit
    • Copy of Hospital bills
    • Copy of X-ray report or Laboratory report including Histopathology Report if claiming for First Cancer Benefit
    • Copy of Inpatient Discharge Summary Report from government restructured hospital; OR Hospital Clinical Summary Report or Doctor’s memo from the private hospital/private specialist.
      (If the said report is not available, then Attending Physician Statement is required)

  • Medical Reimbursement Benefits eg. Outpatient Cancer Treatment/Kidney Dialysis/Inpatient Major Organ Transplant Benefits
    • Original Hospital bills and/or Clinic bills
    • Copy of X-ray report or Laboratory report (if any)
    • Copy of Inpatient Discharge Summary Report from government restructured hospital; OR Hospital Clinical Summary Report or Doctor’s memo from the private hospital or private specialist. If the said report is not available, then Attending Physician Statement is required.
    • Copy of Settlement letter from Insurer/ Employer, and copy of Hospital bills and/or Clinic bills (if claiming balances from AXA)
Premier Care Policy
Please submit:
  • Daily Hospital Cash Benefit including Daily Room & Board and Intensive Care Unit (ICU), and/or
  • Surgical Benefit
    • Original Hospital bills or Clinic bills
    • Copy of X-ray report or Laboratory report including Histopathology Report if claiming for First Cancer Benefit
    • Copy of Inpatient Discharge Summary Report from government restructured hospital; OR Hospital Clinical Summary Report or Doctor’s memo from the private hospital/private specialist.

MumsCare / MumsCare Plus Policy

    • Copy of Inpatient Discharge Summary Report from government restructured hospital; OR Hospital Clinical Summary Report or Doctor’s memo from the private hospital/private specialist.
      (If the said report is not available, then Attending Physician Statement is required)

  • Congenital Illness Benefit
    • Attending Physician Statement for Child & Laboratory report (if any)

      Please call our Customer Service hotline for the APS to be submitted (based on the type of congenital illness that you are claiming for).
  • Pregnancy Complications Benefit
    • Attending Physician Statement for Mother & Laboratory report (if any)

      Please call our Customer Service hotline for the APS to be submitted (based on the type of pregnancy complications that you are claiming for).

Submit the completed Claim Form and Supporting Documents

You can submit your claim form and Supporting documents through any 1 of these channels:

Additional Information
  • After Claim Submission

    We settle most claims within 7 working days after we have received the claim form and supporting documents.

    Sometimes, we may require more time to assess claims which we are seeking clarification on or internal discussion. For such cases, we will keep you informed of the claims progress.
