Please follow the STEPS below for Hospitalization Claim (applicable for Prime Care, Premier Care, MumsCare/MumsCare Plus policies).

Step 1

Complete Claim Form

  • Submit the claim within 180 days from the event date.
  • Complete Claim Form (Outpatient and Dental Claim Form or Inpatient Claim Form) **
    * For Inpatient Claim Form, Part II of the form is to be completed by the Medical Practitioner at the Policyholder’s expense.
  • Submit Claim Form with supporting documents (refer to Step 2)



Step 2
Submit Supporting Documents

Please note that the list of documents below is not exhaustive and that other documents may be requested if necessary.

Outpatient and Dental Claim
Please submit:
  • Amount up to SGD 1,000.00 (email submission to ops.tpa@aa-international.com.sg)
    • Copy of final itemized Hospital bills and/or Clinic bills and proof of payment.
    • Copy of doctor’s prescription for medicines purchased at an external pharmacy
    • Copy of diagnostic test result (Laboratory result, X-Ray, etc), Copy of Hospital Clinical Summary Report or Doctor’s memo from the private hospital/private specialist. (If the said report is not available, then Attending Physician Statement is required).
    • Copy of Settlement letter from Insurer/ Employer (if claiming balances from AXA)
  • Amount above SGD 1,000.00 (by post/by hand)
    • Original Final itemized Hospital bills and/or Clinic bills and proof of payment.
    • Copy of doctor’s prescription for medicines purchased at an external pharmacy
    • Copy of diagnostic test result (Laboratory result, X-Ray, etc), Copy of Inpatient Discharge Summary Report from government restructured hospital; OR Hospital Clinical Summary Report or Doctor’s memo from the private hospital/private specialist. Inpatient Claim (by post/by hand)
Please submit:
  • Original Final itemized Hospital bills and/or Clinic bills and proof of payment.
  • Copy of doctor’s prescription for medicines purchased at an external pharmacy
  • Copy of diagnostic test result (Laboratory result, X-Ray, etc), Copy of Inpatient Discharge Summary Report from government restructured hospital; OR Hospital Clinical Summary Report or Doctor’s memo from the private hospital/private specialist. (If the said report is not available, then Attending Physician Statement is required).
  • Copy of final itemized medical bills and Copy of Settlement letter from Insurer/ Employer (if claiming balances from AXA)


Step 3

Submit the completed Claim Form and Supporting Documents

You can submit your claim form and Supporting documents through any 1 of these channels:

  • ***By Email to ops.tpa@aa-international.com.sg for the following claims : -
    • (i) If you are claiming for outpatient and dental claim upto SGD 1,000.00
    • (ii) If you are claiming the balance of your hospital/clinic bill from AXA Life
    • (iii) If you are claiming for cash benefit (This is payable for eligible in-patient Treatment only when the Life Assured receives Treatment, within the Area of Cover, provided no other cost is borne by Us. No other benefit will be payable in respect of the period for which the cash benefit has been claimed)
    *** For submissions via Email, you do not need to submit the original claim form and supporting documents to us. However, please do keep the original documents for at least 7 years in the event such documents may be required by our Company.
  • By Post to :-

  • For claim amount above SGD 1,000.00, please submit the original final itemized hospital bills and/or clinic bills and proof of payment by hand to your Financial Consultant, OR by post to the address stated above.

Additional Information
  • After Claim Submission

    We settle most claims within 7 working days after we have received the claim form and supporting documents.

    Sometimes, we may require more time to assess claims which we are seeking clarification on or internal discussion. For such cases, we will keep you informed of the claims progress.
