What to do in the event of a claim?
Please submit the completed Personal Accident Claim Form together with the supporting documents for processing.
  • Duly Completed Personal Accident Claim Form
  • A copy of the Death Certificate
  • Autopsy Report, Coroner’s Report, etc.
  • Letter of Administration
Permanent Disablement and Medical Expenses
  • Duly Completed Personal Accident Claim Form
  • Medical Report from attending doctor
  • Original Medical Bills

Before you begin our e-Claims Notification, have you done the following?
  1. Read our claims procedure in our website.
  2. Notify your agent/broker immediately of the claim or call us at our hotline no. 1800 880 4741 for further notice.
If you have done so, you may proceed to file your e-Claims.
