Simple step-by-step procedure when making a claim...
Step 1: Complete the claim form
Complete the appropriate claim form (as available in the Claim Forms tab) depending on which claim is to be made. All details with respect to member’s ID card/passport number, policy/membership numbers, name of policyholder, etc. should be duly filled.
Step 2: Obtain original documents, medical reports & invoices
Based on the claims made, in case of hospitalization or medical related claims, please submit all original medical receipts together with invoices/bills, original or photocopies medical report (issued by the attending registered physician), and appropriate documents.
For admission to Government/Restructured Hospitals (Alexandra, Changi, KK Women’s & Children, National Univerisity, Singapore General Hospital and Tan Tock Seng), this includes:

A copy of the Inpatient Discharge Summary given to patient upon discharge.

A copy of the Day Surgery Authorization Form signed by the patient before surgery.

A copy of the Referral Letter from a General Practitioner, if any.

The original final Summary Bill and original detailed final Hospital Bill.

Original Outpatient Bills/Receipts for any Pre-Hospitalisation / Pre-Surgery and Post-Hospitalisation follow-up bills for expenses incurred.
For admission to a Private Hospital (EastShore - now known as Parkway East, Gleneagles, Mount Alvernia, Mount Elizabeth, Raffles, Thomson Medical Centre and Day Surgery Centres/Clinic), this includes:

Medical Certification of Treatment to be completed by the attending physician/surgeon.

A copy of Referral Letter from a General Practitioner, if any.

The original final Summary Bill and original detailed final Hospital bill.

All other original Outpatient Hospital / doctor's bill and receipts related to admission/surgery.
Step 3: Submit required documents along with the claim form
Once the claim form is completed and all related documents are in place, submit them to AXA Insurance Singapore by post to:

What to do when requesting for a Letter of Guarantee
Procedure for Government/Restructured and Private Hospitals
The patient is required to complete the Release of Medical Information Form and arrange for the Pre-Admission Form to be completed by the attending doctor and email the completed forms to healthops@axa.com.sg or fax it to +65 6880 4570 at least 5 working days before the scheduled surgery.
We will verify the policy coverage and issue a Letter of Guarantee if the medical condition/illness is covered under the policy. If the medical condition/illness is not covered, we will inform the hospital and the patient will have to bear the cost of the medical bills upon discharge.
The patient is required to complete and sign the Medisave Authorisation Form in order for the Letter of Guarantee to be valid.
For Overseas Hospital
Please settle the medical expenses with the hospital and seek reimbursement from us.
